Forms are on this page in PDF format. Simply click on the form, print it out, and return it to the appropriate office. Some computers may also allow you to fill out the form online, at which point you can print and mail, or email the completed form to the correct staff member.
COMPLIANCE FORMS
Student-athletes may have the need to fill out several of the following forms regarding NCAA compliance throughout their time at Fairfield. The sports medicine department has several forms for student-athletes to fill out at the beginning of each year. Several forms are required for All Student Athletes, while others are for newcomers (Freshmen & Transfers) or returnees only. Please print out and complete the form and return to:

The Sports Medicine department is also responsible for the education of student-athletes and coaches on the dangers of drug use, including illegal and legal drugs, and supplements.
